This isn’t strickly health care, following is a brief summary of the services desired. Three times per week from 1pm to 4 or 5pm (must be afternoon but times aren’t set in stone). Elderly couple, you would help organize or put things away, keep house tidy, laundry, and cook a simple meal sometimes (particularly a meal that can be just reheated next day etc). Every conceivable task could be listed but in sum the job is to keep the household neat and assist where needed. The wife recently had surgery.
